Job Summary:

Under the supervision of the Director of Patient Care Services and Rehab Director, assesses, plans, implements and evaluates the care of all assigned patients. Assesses patient needs, plans and implements patient care and evaluates patient’s response to treatment. Manages care within therapeutic environment and directs the delivery of nursing care by other members of the nursing treatment team. Provides comprehensive nursing care and rehabilitation nursing staff leadership.

Cares for adolescents, adults and geriatric patients with a variety of medical/surgical problems at various developmental levels. Patients are from both traditional and nontraditional family structures with a variety of sociocultural and educational backgrounds.

The Registered Nurse is responsible to assess, plan, implement and evaluate nursing care for assigned patients. The Registered Nurse demonstrates competency in nursing practice as outlined in the Nebraska Nurse Practice Act. The Registered Nurse must be trained to provide care for patients in their primary department.

Essential Duties:

  • Performs procedures and treatment and uses equipment according to Nursing Service policies and procedures; e.g., administers IV medication, performs endotracheal suction, inserts nasogastric tubes and so forth.

  • Assigns, coordinates and/or directs care given by other nursing staff in the Director's absence. Identifies when other unit personnel need assistance and offers help.

  • Orients new personnel and serves as a resource person by answering questions, instructing in policy and procedures and so forth. May be asked to give periodic tours and information regarding Acute Inpatient Rehabilitation.

  • Record Functional Independent Measures (FIM) scores throughout the shift for all patients.

  • Utilizes the nursing process to assess and prioritize patient needs.

  • Formulates, implements and evaluates the patient’s plan of care. Adjusts plan of care in consultation with the Charge nurse as appropriate and implements plan.

  • Uses basic communication skills to provide care to assigned patients/ families. Relays pertinent information through appropriate documentation and referral to other health care team members such as medicine, dietary and support staff. Initiates discharge planning working in collaboration with the appropriate staff.

  • Initiates appropriate nursing action in crisis or emergency situations.

  • Carries out physician orders.

  • Provides continuity of care to meet the patient’s changing needs.

  • Records information on the patient record in accordance with charting guidelines.

  • Utilizes time management principles and techniques.

  • Recognizes changes in patient status based on interpretation of assessments.

  • Recognizes examples of use of evidence/research based practice- at performance appraisal presents journal article supporting specific approach to a nursing care problem.

Qualifications

Minimum Knowledge, Skills and Abilities Required

Education

  • Knowledge of nursing theory and practice acquired through completion of an accredited school of nursing to be eligible for licensure as a Registered Nurse.

  • Will complete Bachelor of Science in Nursing (BSN) within five years of hire from an accredited nursing program (if an ADN or Diploma RN)

Licensure and Certification

  • Current Nebraska license as a Registered Nurse.

  • Current certification in Basic Life Support.

  • After 2 years of employment, strongly encouraged to pursue certification in specialty area.

Experience

  • A minimum of 6 months acute rehabilitation or acceptable related training and experience is preferred to acquire and effectively utilize knowledge of hospital, unit, departmental procedures and policies to meet quality and quantity standards.

Skills

  • Analytical skills necessary to develop and implement effective nursing care and problem-solving measures for patient's physical, psychosocial and spiritual well being.

  • Interpersonal skills necessary in order to gather and exchange information with physicians, patient, family, visitors and other hospital personnel. Ability to work with multiple departments to continually improve patient satisfaction and to promote a healing environment.
