Catholic Health Initiatives Clinical Pharmacist in Lexington, Kentucky
The Staff Pharmacist provides safe and appropriate pharmacy services to home care infusion patients and coordinates and assists in managing / participating in pharmacy services under the direction of the Pharmacy Manager. The Pharmacist is responsible for patient prescriptions, patient clinical management, compounding, quality assurance, receiving physician orders and medical consultation.

Participates in defining and maintaining AmeriMed’s scope of service.
Communicates knowledgeably on the scope of services available through AmeriMed.
Assesses patients’ suitability for home infusion care in accordance with AmeriMed’s policies and procedures. Knowledgeable of medical conditions and prescribed medication therapies suitable for home care services.
Monitors and evaluates patient-specific pharmacy care plans for the pharmacotherapy of home infusion patients. Collects and organizes all patient related data needed to prevent, detect and resolve medication-related problems. Documents outcomes in CPR+, completes company occurrence reports. Designs and modifies pharmacy care plans based on evaluated information and makes recommendations for continued monitoring when appropriate.
Receives physician prescription orders and evaluates the appropriateness of orders for dosage, drug choice, potential drug interactions, drug-food interactions, route of administration and stability.
Ensures medication specific laboratory tests are ordered and results received. Responsible for the retrieval, review and consultation with physicians and nurses of ordered labs. Utilizes critical lab, critical value report.
Manages the use, maintenance and troubleshooting of all pharmacy equipment. Ability to accurately program, maintain and troubleshoot all IV medication related equipment.
Prepares and dispenses medications, selecting cost effective and patient safe and appropriate methods of infusion, following AmeriMed’s policies and procedures.
Allows for the continuity of pharmaceutical care from pharmacy through the delivery process. Ability to communicate pertinent information to the patient/caregiver and nursing department.
Provides medication-related education to AmeriMed staff, nurses, physicians, and patients/caregivers.
Assists the physician, nurse, patient/caregiver and AmeriMed staff with continued technical and clinical support. Provides accurate, concise and timely responses to clinical questions from health care providers and patient/caregivers.
Assists in the training and supervision of pharmacy technicians, pharmacy externs and delivery personnel.
Assists in providing in-service programs on any aspect of drug therapy.
Assists the Pharmacy Manager in implementing new policies and procedures.
Assists in the maintenance and support of all necessary paperwork required by the state and federal agencies as well as Joint Commission.
Accurately documents all communication with physicians, nurses, patient/caregivers and other medical personnel in the electronic medical record/patient chart.
Properly utilizes pharmacy support staff. Prioritizes the workload and organizes the flow in order to maintain efficiencies.
Ensures the accuracy of the work produced by the pharmacy support personnel.
Participates in interdisciplinary patient care conferences.
Participates in the on-call rotation.
Participates in branch quality assurance programs.
Participates in the pharmacy team meetings.
Follows all AmeriMed policies and procedures.
All other duties as assigned.
REQUIREMENTS OF THE POSITION
Registered pharmacist/State licensure in state(s) serviced.
One year hospital pharmacy experience required.
Experience with and knowledge of IV admixture procedures.
Excellent communication skills: oral and written.
Flexible and able to work with a multidisciplinary team.
Excellent time management skills and able to work independently.
Ability to function well as part of team.
