LVN Intake Coordinator Part time


Position Summary:

The Intake Coordinator effectively screens all potential admissions and transfers for the appropriateness of acceptance and appropriate level of care. Utilizes nationally recognized criteria for this screening and request the appropriate level of care for each patient screened. Communicate effectively with internal and external customers to ensure a smooth transition of care of patients entering the CHI St. Luke’s Memorial system. Facilitates transfers in from outlying facilities. Performs status changes for patients moving to various levels of care within the facility and monitors ongoing appropriateness of status of observation patients.

Position Responsibilities:

  1. Interpret information in the medical record and apply it to nationally recognized criteria to determine appropriate status and level of care

  2. Request additional information as neededto ensure that the medicalrecord supports the requested level ofcare

  3. Possess the ability to negotiate and communicate with other departments, physician practice groups,3rd party reviewers and outside facilities

  4. Review observations for continued appropriateness of observation, change to inpatient or readiness for discharge

  5. Facilitate efficient transfer of patientsinto CHI St. Luke’s Memorial

  6. Manage time and resources effectively

  7. Perform 100% review of anticipated admissions utilizing InterQual criteria to assess for appropriate level of care assignment and appropriate admission status

  8. Ensure that an appropriate status order is present prior to bed assignment and initiate the Medicare Certification Process

  9. Obtain additional information from physicians including telephone orders as needed to ensure that adequate information is available to support the requested level of care

  10. Ensurethat pre-authorization information is obtained from physician officesfor scheduled procedures

  11. Serves as a resource to patients, physicians, Administration and hospital employees regarding issues related to patient classification and reimbursement.

  12. Refers inappropriate Observation or Admissioncases to the RN Case Manager for secondary review

  13. Assist in facilitating transfersfrom outlying acute care facilities into a CHI St. Luke’sMemorial

  14. Providereal time interventions to prevent delaysand ensure compliance and revenue integrity with health care regulations

  15. Function as a resourceto the ED by communicating new information regarding coordination of care,Case Management, managed care and the continuum of care

  16. Participates in Case Management or other projects according to departmental and organizational monitors.

  17. Performs basic administrative tasks related to the job to maintain accurate records and to ensure worker accountability/productivity.

  18. Maintains a highly acceptable level of professional conduct and respect for medical staff, coworkers and hospital staff to foster a desirableimage for the institution.

  19. Abide by all hospital/departmental policies/procedures including work hours, scheduling and other criteria for expected daily operations of department.

  20. Maintains strict confidentiality in dealing with all patient-related activities and other sensitive physician/hospital issues by strictly adheringto hospital confidentiality of information policies.

  21. Demonstrates ability to prioritize multiple work assignments in order to accomplish assignedworkload.

  22. Abides by all hospital/departmental policies and procedures assigned by the Manager of Case Management including work hours, scheduling and other criteria for the expected daily operation of department.

  23. Performs other dutiesas may be assigned in order to assure that departmental objectives are fulfilled.


Minimum Qualifications:

Education Level

  • High School Diploma/GED

  • Graduate of an accredited vocational nursing school


  • Licensed in the Texas as a Vocational Nurse (LVN)

  • Demonstrates excellent interpersonal skills both written and verbal

  • Basic Computer skills – familiarity with Windows based software programs
