Job Summary:

This position is responsible for implementing the Community Link Program by serving as direct support to patients in screening for, understanding, and making appropriate referrals for basic social needs (e.g., transporting, food, housing) that directly impact the patient’s short- and long-term health. The Advocate will be an integrated part of the clinic care team and will work directly with patients, support a team of Student Liaisons, and build and maintain relationships with community resource partners.

Essential Duties:

  • Build and Maintain Strong Clinic and Partner Relationships

  • Participate as a full member of the clinic team, including attending team meetings and acting as an advocate for Community Link

  • Build and maintain relationships with local network of resource providers including: identifying opportunities to integrate community resources within clinic settings as needed; attending local coalition and partner meetings as needed; ensuring community partners needs are met through Community Link referral process; identifying gaps in community resources based on Community Link client needs

  • Support Community Link integration into the clinical setting through strategies such as implementation of screening and referral systems and training for clinic staff to screen for social determinants of health and refer patients to Community Link

  • Communicate innovations and challenges to Program Manager

  • Support Student Advocates and Leaders

  • Function as on-site support to facilitate Student Liaison work with patients, and help triage patients back to clinic care team as needed

  • Support coaching and feedback on student performance as needed with Program Manager

  • Work with a caseload of clients professionally and effectively

  • Work with families and members on needs that may or will affect the member’s health, including transportation issues, financial concerns, etc.

Hours: M-F 8 a.m. to 5 p.m. with occasional evening and weekend hours for community events. Prefer a candidate who is Bilingual (English and Spanish)

Qualifications

KNOWLEDGE/SKILLS/ABILITIES:

Qualifications

  • Experience working in the community in a volunteer or paid capacity related to work within community service agencies, care management, or health care delivery. May include advocacy, community organizing, or other similar work.

  • Proven knowledge of landscape of public benefits and community resources and how to facilitate individuals’ access to those resources.

  • Demonstrates ability to work in a multi-cultural setting with a diverse population and exhibit cultural competency.

  • Demonstrates ability to read and comprehend patient information from an electronic record system, as well as record accurate and appropriate information in the system. Experience with electronic medical records preferred.

  • Knowledge and skills in basic word processing required.

  • Ability to speak multiple languages preferred.

  • Ability to have a flexible schedule as program needs arise (may require evenings and weekends).
