Job Summary/Essential Duties:

This position contributes to the mission and vision of Alegent Health by assisting in meeting the psycho-social, rehabilitative, financial and discharge planning needs of patients and families requiring continuing care or adjustment following discharge and assisting patients/families with personal or environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Serves as a consultant to Care Managers and Regional Outreach Nurses in identifying, organizing and solving problems regarding discharge planning.

  • Gathers relevant information systematically; considers a broad range of issues or factors; grasps complexities and perceives relationships among problems or issues; seeks input from others; uses accurate logic and analysis.

  • Relates to people in an open, friendly, accepting manner; shows sincere interest in others and their concerns; initiates and develops relationships with other as a key priority.

  • Develops effective give and take relationships with others; understands the agendas and perspectives of others; recognizes and effectively balances the interests and needs of one’s own group with those of the broader organization.

  • Speaks clearly and expresses self well in groups and in one to one conversations.

  • Ensures 100% of patient population is screened for discharge planning and biopsychosocial needs within 72 hours of admission. Counsels and interviews patients/family members and conducts an appraisal of social, emotional and economic complications in order to provide the physician and the health care team with recommendations and information which will assist in developing an overall plan for the patient’s care upon consultation or, if functioning, in a selected area.

Hours: Monday-Friday 8a-4:30p, PRN As needed



Preferred: Acute care experience as a social worker.


Required: Bachelor of Social Work.

Must have a CSW Social Worker to practice in the State of Nebraska or have the PLMHP/LMHP or PLCSW/LCSW status



Attends ongoing continuing education seminars related to social work practice.

Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient’ relative to his or her age-specific needs. Expertise regarding health and supportive services resources.

Ability to develop an accurate biopsychosocial assessment.
