Job Summary

This position contributes to the mission and vision of Alegent Health by assisting in meeting the psycho-social, rehabilitative, financial and discharge planning needs of patients and families requiring continuing care or adjustment following discharge and assisting patients/families with personal or environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Serves as a consultant to Care Managers and the multi-disciplinary team in identifying, organizing and solving problems regarding discharge planning.

Essential Duties:

  • Gathers relevant information systematically; considers a broad range of issues or factors; grasps complexities and perceives relationships among problems or issues; seeks input from others; uses accurate logic and analysis.

  • Collaborates between the Emergency Departments, community and the behavioral inpatient units to facilitate timely access to appropriate unit for behavioral patients.

  • Participates in all Bed Huddle Calls to review the needs of the community and availability of beds within the Alegent System of Care.

  • Assists with transition of patients from ED to Unit by support with assessments or support of the unit to provide Soarian Assessment, precerts,

  • Assist in verifying insurance benefits after level of care has been determined

  • Liaison with Alegent psychiatrists and other providers to assure that patient transfers are appropriate and processes are smooth and without unnecessary delay.

  • Relates to people in an open, friendly, accepting manner; uses mission, vision and core system values in conducting daily contact with patients, families peers, physicians and all stakeholders.

Hours: 72 hours in 2 weeks, every other weekend commitment




Two years acute care experience as a social worker; experience in a behavioral or geriatric setting preferred.


Required: Master of Social Work and Current Unrestricted Certification to practice as a Social Worker in the State of Nebraska/Iowa. Certified Master Social Worker (CMSW), Licensed Master Social Worker – IA (LMSW), Licensed Mental Health Practitioner (LMHP), Provisional Licensed Mental Health Practitioner (PLMHP), required.

Crisis Prevention Intervention (CPI) required within 6 months of hire

BLS or BLS instructor certification is required upon hire. Acceptable credentialing bodies and certifications include the following: American Red Cross: CPR/AED for the Professional Rescuer, American Safety and Health Institute: CPR, PRO, must say “Professional Level-with AED”, and American Heart Association: Basic Life Support for Healthcare Providers.


Attends ongoing continuing education seminars related to social work practice.

Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs.

Expertise regarding health and supportive services resources.

Ability to develop an accurate biopsychosocial assessment and translate assessment information into an active treatment plan in conjunction with collaboration of other treatment team members.
