Overview

We are looking for a PRN Care Transition Nurse to join our amazing team at Amerimed in West Chester!

PRN Care Transition Nurse - Infusion (Kettering & Good Samaritan hospitals)

Responsibilities

  • Following referral of a patient to CHI Health at Home’s, assists referral planners in coordinating quality home care infusion services for clients.

  • After referral, communicates with attending physicians to obtain verbal or written orders for home care services.

  • Assists in ordering equipment, oxygen, IV’s, etc., as required to provide proper home care for the patient referred to CHI Health at Home.

  • After referral, determines individual patient needs for services, referrals, training and provides patient instructions on home care equipment and/or procedures, etc.

Qualifications

  • Graduate of an accredited school of nursing.

  • Current RN or LPN license to practice in the state serviced.

  • Two years of appropriate nursing experience is required. Hospital and/or home care infusion nursing experience is preferred.

  • Experience as a Care Transition Nurse or similar role, sales or marketing, or customer service preferred.
