Medication Reconciliation Tech- Pharmacy-Mercy West Lakes Medical Center-FT 1.0 (Days)1/3 wknd



Provide technical support in obtaining,

verifying for accuracy, and recording initial medication histories from

patients &/or their designated family/caregivers upon presentation to Mercy

patient care areas. Includes conducting

patient &/or family interviews focused on obtaining current medication

regimens including prescription and over-the-counter (non-prescriptoin) medications,

as well as vitamin and herbal supplements.

Perform other functions of a pharmacy

technician registered in the State of Iowa and within the scope of Iowa law as



  • Successfully complete MedicationHistory Pharmacy Technician Competency/Skill Set

  • Interview patients &/or theirdesignated family/caregivers as soon as possible after presenting forcare to Mercy clinical areas (may include the Emergency Department, as well asinpatient and outpatient areas).

  • Obtain & record in theelectronic medical record a current list of medications to include:prescription medications, over-the-counter (non-prescription) medications,vitamin and nutritional supplements, as well as the source(s) for thisinformation.

  • Recording should include thefollowing: name/location of patient’sambulatory pharmacy, medication/supplement name, dosage form, strength, dose, day/timeof most recent dose, and source(s) of the information being recorded.

  • Attempt to clarify as manyuncertainties and compile as complete a medication history as possibleutilizing patient and family/caregiver information, ambulatory pharmacyrecords, clinic records, long-term-care facility records and information fromprevious hospital encounters.

  • Obtain & record in theelectronic medical record an allergy history for each patient. In doing so,clarify & record the nature of the allergic reaction when necessary.

  • Consult with a pharmacist in allcases of unresolved uncertainties related to a patient’s medication &allergy history.


  • Perform other functions asdescribed in the Mercy Medical Center Pharmacy Technician job description asneeded.



  • High school diploma or GED.

  • High level of verbal and writtencommunication skills.

  • Aptitude in math and science.

  • Typing and computer skillsrequired

  • Pharmacy Technician Certificationrequired.

  • Minimum of 3000 hours of pharmacytechnician experience required.
