American Nursing Care is looking for an RN Population Health Specialist. The Population Health Specialist is a licensed registered nurse who consistently demonstrates the skills, knowledge and confidence required to provide exceptional clinical care to moderate and high risk complex patients. The Population Health Specialist will lead the multidisciplinary team in developing, implementing and monitoring a holistic, patient-centered, outcome driven plan of care. This position ensures competence in managing the patient episode of care using best standard practices from start of care to hand off. The competent Population Health Specialist will be experienced, effective, resourceful, and knowledgeable.

Responsibilities

  • Provides care following the plan of treatment and patient care plan, and establishes a care plan for continued needs based on the client assessment.

  • Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.

  • Reports changes in patient condition to appropriate personnel and the attending physician, and participates in coordination of services with other disciplines and community resources as needed.

  • Administers, teaches, supervises, and coordinates nursing care to patients with guidance of supervisory staff.

  • Provides paraprofessional supervision as required by policy and regulation.

  • Provides case management by collaborating with physicians and multidisciplinary team members; and by providing physical and psychological support to patients and their care givers; supervising/mentoring assigned team members.

  • Performance Improvement (PI) Program Coordination including; assisting with the overall development and implementation of the PI plan, assisting in the identification of goals and related patient outcomes, coordinates participation and reporting of activities and outcomes.

  • Completes documentation to comply with regulatory, operational and financial requirements.

  • The RN-Population Health provides exceptional clinical care to moderate and high risk complex patients. Ensures competence in managing the patient episode of care using best standard practices from start of care to hand off. Experienced, effective, resourceful, and knowledgeable.

Qualifications

  • Required Minimum Education Graduate of an accredited school of nursing.

  • One year of RN nursing experience.

  • Licensed to practice in the state serviced.

  • Must obtain and maintain current CPR or BLS Certification.

  • Compliant with appropriate state certification/registration and CHI Health at Home requirements as necessary.
