Reports directly to the Operations Director, Care Management through the Lead. Works
collaboratively with all staff to achieve the goals of Alegent Health and the Care Management Department.
Additional responsibilities for this health care role include:

Assists with the development, assessment, implementation, and monitoring of a comprehensive plan of care for identified populations by coordinating an interdisciplinary team approach to service and resource delivery. The assessment will include patient goals, as well as biophysical, psychosocial, environmental, and discharge planning needs. Maintains flexible work hours to accommodate patients and physicians which often includes long hours and unscheduled shifts for emergency cases and peak periods of activity.
Three years of progressive clinical experiences as a registered nurse preferred. Two years experience with disease specific population for a specialized care manager is preferred. Strong emphasis on critical thinking, diagnostic testing and behavioral procedures is highly desirable.
BSN or RN plus Bachelor’s degree and unrestricted license in the state of Nebraska
