advanced psychosocial theory and methods to individuals of all ages as well as

couples, families and groups with appropriate regard for the complexity of

given problems. Responsibilities encompass the provision of advanced

evaluative, preventive, therapeutic, restorative and personal care services to

assist in meeting the complex psychosocial needs of Mercy patients and their

families, thus enabling them to better utilize medical care resources in order

to achieve their optimal level of health. May include providing consultative

services to rural hospitals home care agencies and long-term care facilities.

As assigned, provides social work services to patients at any and all points on

the continuum of care, from pre-admission to post-discharge follow-up. May

interact directly with persons of diverse ages.


  • Obtains social history information and identifiespatient needs by interviewing the patient/family, reviewing the medical record,conferring with physicians and appropriate medical center staff and contactingother information sources as necessary and appropriate. Analysis and evaluationof patient situation

  • Consistently reflects advanced competency inpsychosocial assessment and diagnosis. Assesses problems appropriately in termsof scope, consequences and needed interventions.

  • Develops individualized psychosocial case plansbased on assessment and in conjunction with other members of the health caredelivery team. Implements plan and monitors patient's progress.

  • Arranges for and/or coordinates a broad spectrumof discharge planning services. Consults with physicians, medical center staff,patient/family to determine patient's post-hospital needs and plans. Providespatient/family with information on options/resources available and strategiesfor utilizing these resources. Assists with all necessary referrals.

  • Demonstrates ability to differentiate betweenemergency and non-emergency tasks and responds accordingly. Provides immediatesupportive counseling and necessary assistance to patients/families dealingwith a crisis or emergency situation. Assists with referrals to communityresources when appropriate.

  • Provides psychosocial counseling/treatmentservices to restore patients and families to optimum social and physical healthadjustment within their capacity.

  • Provides factual information and materials topatients/families and others. Possesses knowledge of public and private welfareand health agencies available to serve the community. Makes referrals promptlyto appropriate agencies for services suitable to meet patient needs.

  • Screens patients for financial assistance andhelps clarify insurance benefits. Supports patients and families in theirefforts to address and cope with the economic stresses brought on by theirillness/injury.

  • Provides clinical and administrative social workconsultation to physicians, allied health professionals, long-term carefacilities and other medical centers through patient evaluations, caseconferences, in-service programs and educational presentations.

  • Maintains liaison with community health andsocial service agencies, establishes effective professional relationships bypersonal/telephone contact and correspondence.

  • Provides for the emergent needs of patients andfamilies through participation in the department's on-call program.

  • Documents social work activity in patient'smedical record. Prepares and maintains records/reports in a timely andqualitative fashion.

  • Recognizes the role of a social worker in thedepartment and how it relates to the overall function of the hospital regardingpatient care.

  • Demonstrates leadership in problem solving andprogram development.



  • Master's degree in social work, required.

  • Two years experience in medical social work is preferred.

  • Understands medical terminology

  • Knowledgeable regarding clinical interviewingtechniques.

  • Knowledgeable regarding social workprinciples, standards and ethics.

  • Basicunderstanding of Medicare and Medicaid regulations and child/dependent adultlaws and regulations are desired.

  • Proof ofcompletion of Mandatory Reporter abuse training specific to population served withinthree (3) months of hire.
