Care Transition Nurse I - VNA Health at Home Louisville Metro

VNA Health at Home has been providing compassionate care for over 125 years and is the oldest home health provider in Kentucky. Named as HomeCare Elite™ for the eleventh consecutive year. VNA Health at Home offers exceptional care and a variety of services, tailored to meet the specific and unique needs of our patients.

Responsibilities:

  • Follows referral of a patient to VNA Health at Home, assists referral planners in coordinating quality home care services for clients.

  • Upon patient choice of VNA Health at Home, communicates with attending physicians to obtain orders for home care services.

  • Assists in ordering equipment, oxygen, IV’s, etc., as required to provide proper home care for the patient referred to VNA Health at Home

  • Upon patient choice of VNA Health at Home determines individual patient needs for services, referrals, training and provides patient instructions on home care equipment and/or procedures, etc.

Qualifications

  • Graduate of an accredited school of nursing.

  • Current RN or LPN license to practice in the state serviced.

  • Two years of appropriate nursing experience is required. Hospital and/or homecare nursing experience is preferred.
