RN Case Manager - Lufkin (Full time)


Position Summary:

Under the supervision of the Care Management Director, the RN-Case Manager coordinates patient care between patients, caregivers, and the multidisciplinary team; collects and evaluates information to facilitate care by assuring that admissions, continued stays, and ancillary services are medically necessary, cost-effective, and providing the appropriate setting to qualify for reimbursement. The RN-Case Manager intervenes when necessary to assure that case is care is coordinated and timely. Utilizing a collaborative approach, the RN-Case Manager maintains current knowledge of regulatory requirements and programs related to health care. Assists patients with basic patient care needs. Responsible for proper maintenance of equipment, accreditation, and Departmental Performance Improvement. Displays professional, safe conduct in all interactions. Maintains positive working relationships with staff, other departments, physicians and peers. Displays conduct in support of CHI St. Luke’s Health Memorial Mission & Vision.

  11. Documents on the day of admission, or within the first business day, a clinical assessment and anticipated discharge plan that addresses use of the full continuum of care

  12. Provides plan of care updates to providers inside and outside of the hospital, working with patients and caregivers.

  13. Continually assesses and modifies the plan of care based on clinical need and patient preference.

  14. Manages each patient’s plan of care and throughout the stay.

  15. Leads interdisciplinary rounds to coordinate care with the care team, and to address changes in the discharge plan.

  16. Consults with appropriate physicians on plan of care and discharge.

  17. Remains current of Medicare and Medicaid laws and guidelines, Medicare Conditions of Participation, managed care contracts, and other legal and regulatory requirements as they relate to health care case management services.

  18. Assures patients are informed of rights and responsibilities as participants in these programs.

  19. Fosters and promotes positive and professional relationships with physicians, physician office staff, other referral sources and insurance companies.

  20. Demonstrates tact and professionalism in working and communicating with patients/families, physicians, other members of the rehabilitation team, and other employees of the healthcare system.

  21. Adheres to CHI St. Luke’s Health Memorial policies, including but not limited to “CHI Values & Ethics at Work”, HIPAA, PHI, ePHI, Privacy Rule, and confidentiality.

  22. Maintains safe work habits; adheres to safety rules; immediately reports any unsafe condition; immediately reports any incident / injury.

  23. Supports department-based goals and contributes to the success of the organization.

  24. Complies with hospital policies and procedures; attends required in – services, education and training.

  25. Performs duties in accordance with that of hospital policies and procedures, and regulatory agency guidelines.

  26. Demonstrates a willingness to follow the chain of command, regularly channels suggestions, criticisms and complaints to the appropriate person.

  27. Performs other duties as assigned.


Minimum Qualifications:

Education Level


Associate's Degree in Nursing (ADN)


Bachelor's Degree in Nursing (BSN)



Registered Nurse with valid Texas license

BLS certification (through an approved AHA program)

Three (3) years of clinical experience.


Three (3) years of acute care case management experience.
