LPN Care Coordinator - Malvern Clinic - Full Time Days


Job Summary:

The Care Coordinator will support assigned physician(s) with patient care and clinical documentation into the electronic medical records in an efficient manner. Perform and direct patient care services including, but not limited to, admission interview, accurate documentation and timely patient flow. Accurately performs ancillary healthcare services while assisting the physician through documentation of specific components of the patient visit and executing specified physician instructions. Ensure priority care is provided to patients in an emergency situation.

Essential Duties:

  • Liaison between the provider and his/her patients:

o Uses appropriate knowledge and skills in the care & interaction with customers across the age continuum.

o Schedules specialty appointments, answers questions, and/or determines the need for provider intervention.

o Schedule and provide patient instruction(s) for tests, treatments and hospital admissions, following correct insurance protocol. (i.e. pre-certification, referrals, chart documentation, etc.)

  • Conducts pre-visit planning activities:

o Reviews schedule 2-3 days in advance of appointment to prepare visit agenda, identifying care opportunities, documents in EHR, and communicates in daily provider huddles.

o Ensure optimization of physician practice by keeping current with EMR flow and templates.

o Ensures all lab and imaging results and/or specialists’ notes are available for the provider.

o Notify patients of lab and diagnostic test results under the direct supervision of the health care provider.

o Discusses any recent care transitions (including recent visits to hospital, ER, specialists, etc.) with the provider.

o Ensure patient is appropriately prepared for care encounter. Prepare and obtain patient medical records and other information/documentation pertinent to patient encounter.

  • Populates patient-reported information into EHR templates for specific disease states and preventive visits:

o Ensures all quality measures, immunizations, and preventive care are up-to-date.

  • Prepares patient for discharge:

o High-risk patients

 Coordinates patient care with Care Management staff, if applicable.

 Educates/coordinates the patient regarding Care Management staff and functions.

 Facilitates the development of Care Plans for high-risk patients.

o Follow-up care coordination

 Schedules follow-up appointment, referrals, procedures, diagnostic imaging and provides appropriate instructions.

o Completes referral process.

  • Post-visit duties

o Schedules specialty appointments, answers questions, and/or determines need for provider intervention.

o Ensures all reports and information are received from specialists.

o Monitors patient adherence to plan of care.

o Encourages patient use of portal, patient profile



  • Education :

o High School Diploma or GED Required

  • Experience: Completion of Medical Assistant program or equivalent training. OR Licensed LPN Certification.

  • Physical Requirements: This job requires, at various times, the following physical activities; climbing, balancing, stopping, kneeling, crouching, reaching, standing, carrying, walking, pushing, pulling, lifting, finger dexterity, grasping, feeling, talking, and hearing, sometimes exerting in excess of 20-100 pounds of force. The worker is required to have close visual acuity to perform daily activities. May be subject to inside environmental conditions, hazards (including electrical currents and exposure to high heat and chemicals), infectious diseases, and may be required to function around prisoners and mental patients.

  • Skills: Ability to identify scope of practice issues, prioritize duties and make effective decisions concerning patient care. Demonstrate communication and interpersonal skills, both orally and in writing. Ability to perform multiple tasks, often under stress. Appropriately applies knowledge, training and skills in the care of neonate, adolescent, adult, and older patients. Utilize good organizational skills. Key boarding skills preferred. Successfully completed BLS
